About the Role

We are seeking a skilled and compassionate Registered Nurse or Registered Psychiatric Nurse to join our team as a Community Access and Transition Nurse. This is a unique opportunity to make a meaningful impact on the lives of individuals in need of healthcare services.

Key Responsibilities
  • Develop and maintain relationships with healthcare providers, community organizations, and government agencies to ensure seamless transitions for clients.
  • Conduct comprehensive assessments of clients' needs and develop individualized care plans to address their physical, emotional, and social well-being.
  • Collaborate with interdisciplinary teams to provide holistic care and support to clients, including those with complex mental health and substance use challenges.
  • Provide education and support to clients and their families on health-related topics, including disease management, wellness, and self-care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in community health nursing.
Requirements
  • Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years recent, related experience in acute or primary care or community health nursing or an equivalent combination of education and experience.
  • Successful completion of Occupational First Aid II (OFA II) (or OFA III techniques for positions at Ford Mountain Correctional Centre)
Preferred Qualifications
  • Knowledge of social determinants of health and their impact on health outcomes.
  • Experience working with diverse populations, including those with complex mental health and substance use challenges.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and mutual respect.
