Job Summary
CarePartners is seeking a skilled Community Health Nurse to join our team in the Owen Sound, ON area. As a Community Health Nurse, you will work with patients in their homes, providing exceptional nursing services and developing meaningful relationships.
About the Role
This is a part-time position that requires flexibility in scheduling, including weekends and weekdays. You will work independently or in cooperation with the Home and Community Care Support Services (HCCSS) to develop and maintain effective Care Plans.
Responsibilities
- Provide exceptional nursing services to patients in their homes, clinics, and other settings
- Develop and maintain effective Care Plans with respect to Community Health Services
- Communicate and liaise with patients, family members, and other members of the interdisciplinary care team
- Perform other clinical duties as required
Requirements
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
