Personal Support Worker

As a Personal Support Worker at VON Canada, you will play a vital role in providing companionship, personal care, and health maintenance services to clients in their homes or communities. You will work collaboratively with the healthcare and personal support teams to ensure clients receive the highest level of care and independence.
Key Responsibilities- Observe and evaluate client needs to determine changes to their care plan.
- Contribute to the development of client care plans under the direction of the Personal Support Program Manager.
- Implement changes to care plans as needed, ensuring client safety and well-being.
- Report changes in client needs or home situations to the supervisor or Personal Support Program Manager.
- Perform personal support tasks, including light cleaning, meal preparation, and household safety monitoring.
- Provide personal care, including assistance with walking, dressing, and hygiene.
- Provide respite care and support to clients' families as needed.
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery.
- High school diploma or equivalent.
- CCA Certification (Nova Scotia) or equivalent.
- Must have a certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
- Standard First Aid Certificate or CPR level C Certificate (if applicable).
- Experience working with elderly or ill individuals or children.
- Strong communication and interpersonal skills.
- Ability to work effectively in a multidisciplinary team.
- Good assessment and problem-solving skills.
- Awareness of community resources and ability to connect clients with them.
