Position Overview
We are currently seeking a Diabetes Educator (Registered Dietitian) to engage with our team on a temporary, full-time basis at our Barrie location. This role is designed as a hybrid position tailored to clinic requirements.
Key Responsibilities
Provide education and guidance to patients on self-management techniques in both individual and group formats, primarily focusing on diabetes and secondarily on other health conditions, including general health and metabolic issues such as weight management and obesity. Collaborate with the multidisciplinary team to create personalized care plans for patients. Document detailed patient interactions within the EMR system. Instruct patients on effective insulin administration, including titration and management, in accordance with medical directives. Educate patients on self-monitoring of blood glucose (SMBG) in alignment with medical directives. Conduct patient follow-ups through various methods, including in-person, email, telephone, and video consultations. Gather comprehensive health histories from patients. Offer consultations to community groups, partners, and team members as requested. Facilitate training for new staff and educators as necessary. Communicate effectively with patients, families, caregivers, and healthcare professionals. Utilize information technology to enhance patient care, including statistical tracking. Stay informed about opportunities to implement new, evidence-based patient assessment and treatment methods. Assist with research initiatives, including providing dietary counseling for participants in studies. Conduct evening virtual workshops as required. Participate in a minimum of two community outreach events annually. Complete annual outreach calls. Support program implementation by reviewing patient charts, entering data, and adhering to program guidelines. Manage administrative tasks, which may be delegated to administrative staff, volunteers, or interns, including rescheduling appointments, creating workshop materials, ordering supplies, tracking patient visits, and entering program data. Engage in Journal Clubs and other collaborative learning opportunities. Contribute to the dissemination of the program's vision. Promote LMC initiatives, both funded and non-funded, including paid and volunteer services. Experience in motivational interviewing and evidence-based coaching techniques is an asset. Health coaching experience is valued, or willingness to undergo training as a health coach.
Qualifications
A valid RD license in good standing with the College of Dietitians of Ontario. Experience in teaching and counseling individuals and groups. Must be pursuing or have obtained CDE designation. A valid driver's license in good standing and access to a personal vehicle.
