Lifemark Health Group is seeking a highly skilled and compassionate Community Occupational Therapist to join our team. As a Community Occupational Therapist, you will work with clients in various settings, including their homes, workplaces, and communities, to help them achieve their goals and regain independence.

Key Responsibilities
  • Conduct comprehensive assessments and develop personalized treatment plans to address clients\u2019 physical, cognitive, and psychosocial needs.
  • Provide in-home and hospital discharge assessments, ergonomic assessments, concussion treatment, cognitive rehabilitation, mental health treatment, and other interventions to support clients\u2019 recovery and return to function.
  • Collaborate with clients, families, and healthcare professionals to develop and implement effective treatment plans.
  • Work with clients to identify and address barriers to participation and engagement in daily activities.
  • Provide education and training to clients and their families on strategies to promote independence and safety.
Requirements
  • Registered and in good standing with the College of Occupational Therapists of Ontario.
  • Clinical experience in areas such as brain injury rehabilitation, mental health, musculoskeletal and orthopedic injury, home safety, ergonomics, and/or return to work.
  • Willingness to provide in-vivo OT treatment in various settings, including clients\u2019 homes, workplaces, and communities.
  • Passion for the profession, commitment to ongoing professional development, and dedication to providing exceptional client care.
  • Strong organizational skills, critical thinking, decision-making, and analytic skills.
  • Excellent written and verbal communication skills, empathy, and ability to build rapport with clients and stakeholders.
