In alignment with the organization's core values and strategic objectives, the Community Support Specialist, as part of a collaborative healthcare team, and under the guidance of the Team Lead or designated clinical supervisor, delivers essential home support services to clients following a predefined care plan.
Responsibilities include assisting clients with daily living activities such as personal hygiene, meal planning and preparation, while fostering maximum independence.
Additionally, the role involves performing delegated tasks for which appropriate training has been completed. A vital aspect of this position is to ensure the health and well-being of individuals and families, promoting independent living within the community.
Work Schedule: : Days, 07:00 to 18:00 (Rotating)
