Key Responsibilities:
  • Collaborate with patients and their families to assess care needs and develop individual care plans
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families, ensuring their values and preferences are respected
  • Balance patient needs with available resources, prioritizing patient-centered care
Requirements:
  • Membership in good standing with the applicable regulatory body
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
  • Excellent interpersonal, communication, and assessment skills
  • Effective time management, prioritization, and organizational skills
