About the Role

We are seeking a highly skilled and compassionate Housing Support Worker to join our Community Homes for Opportunity (CHO) Program. As a key member of our team, you will play a vital role in supporting individuals with serious mental illness to maintain their housing, engage with their community, and achieve their goals.

Key Responsibilities
  • Provide individualized and flexible support to clients to help them maintain their housing and engage with their community.
  • Develop and implement wellness plans to support clients in achieving their goals and maximizing their independence.
  • Collaborate with clients to identify their strengths and needs, and develop long-term goals and short-term objectives.
  • Participate in individual and systemic advocacy initiatives to improve resource access and availability.
  • Work closely with the CHO Housing Administrators and Home Operators to ensure seamless support for clients.
Requirements
  • A degree or diploma in health or social services.
  • Current Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Experience working with adults with serious mental illness in the community, preferably in congregate living environments or Homes for Special Care.
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, and relevant legislation.
  • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with clients.
  • A second language reflecting the local community is an asset.
  • A valid Canadian Full G driver's license and proof of insurance.
  • A satisfactory Vulnerable Sector Screening (Police Check).
