About the Opportunity

The Nova Scotia Health Authority is seeking a highly skilled and dedicated Wellness Facilitator-Physiotherapist to join our Hants Health and Wellness Team (HHWT). As a key member of our interdisciplinary team, you will play a vital role in supporting patients with chronic disease management and wellness through a community development, population health, and collaborative approach.

Reporting to the Health Services Manager, Primary Health Care, you will use your critical thinking and clinical judgment to support patients in both individual and group-based settings. You will work closely with HHWT staff, primary care providers, other healthcare providers, and community groups to maintain awareness of existing supports within the community and surrounding areas.

About You

We are looking for a candidate with a Bachelor's degree in physiotherapy from an accredited university and membership with the Nova Scotia College of Physiotherapists. You must be eligible for membership with the Canadian Physiotherapy Association and have a minimum of one year of experience in each of the following:

  • Working with people with chronic disease in an exercise program or setting
  • Working with community groups/partners building trusting relationships through working together on projects/initiatives/partnerships that benefit the public
  • Individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles

You must also have an understanding of population health, health promotion strategies, and behaviour change principles, as well as in-depth knowledge of local health and community-based services.

Responsibilities

As a Wellness Facilitator-Physiotherapist, your responsibilities will include:

  • Providing patient-centered care and support to individuals with chronic disease
  • Developing and implementing wellness programs and services in collaboration with community groups and healthcare providers
  • Building and maintaining relationships with community partners and stakeholders
  • Conducting individual and group education and facilitation sessions
  • Participating in quality improvement and program planning initiatives
Requirements

To be successful in this role, you must have:

  • A Bachelor's degree in physiotherapy from an accredited university
  • Membership with the Nova Scotia College of Physiotherapists
  • Eligibility for membership with the Canadian Physiotherapy Association
