Job Summary

We are seeking a skilled Registered Nurse to join our Rapid Response Nursing team. As a Rapid Response Nurse, you will work with medically complex children and adults, providing hands-on care and support to ensure a smooth transition from acute care to home care.

Key Responsibilities

  • Screen potential patients for program eligibility in hospital
  • Confirm scheduling of outstanding medical tests and ensure new prescriptions are filled
  • Review medication protocol with patients and caregivers, and answer any questions
  • Facilitate patient follow-up visits with primary care physicians
  • Provide direct care to patients in collaboration with LHIN Care Coordinators or Service Providers
  • Identify patients requiring accelerated assessments and home care services
  • Support LHIN Care Coordinators in developing patient care plans

Requirements

  • Membership in good standing with the College of Nurses of Ontario
  • Registered Nurse (BScN or diploma) in good standing with the College of Nursing
  • Minimum of five years of experience as a Registered Nurse
  • Working knowledge of community resources and roles of healthcare professionals
  • Emergency/critical care and community nursing experience an asset
