Position Overview

As a key member of the Calderdale Council, you will take on the role of Virtual Care Team Leader. This position involves overseeing two teams, each comprising 37 hours of dedicated service. You will play a crucial role in enhancing the health and wellbeing of adults in the community.

About the Role

In this leadership position, you will be responsible for guiding the Gateway to Care and Hospital Integrated Management Care teams. Your expertise will be essential in driving forward our mission to improve the quality of life for individuals and their families. You will work collaboratively with various stakeholders, including the Council, Integrated Care Board, and local health trusts, to ensure that our services are effective and responsive to community needs.

Key Responsibilities

- Lead and manage a team of social care professionals, ensuring high standards of service delivery and compliance with regulatory requirements.
- Participate in case conferences, court hearings, and statutory reviews, providing a social care perspective and making informed decisions.
- Mentor and support staff, fostering a culture of continuous professional development and excellence in practice.
- Develop strong partnerships with local agencies and community organizations to enhance service delivery and support for vulnerable adults.
- Drive innovative approaches to social care, focusing on early intervention, prevention, and promoting wellbeing.

Qualifications

To be considered for this role, you must be a qualified social worker with current registration and significant post-qualification experience. You should possess additional qualifications such as Practice Education or Approved Mental Health Professional status, or be willing to pursue these qualifications within a specified timeframe.
