About the Role

We are seeking a highly skilled and experienced Nurse Practitioner and Clinical Supervisor to join our team at the Lakeview Family Health Team. As a key member of our leadership team, you will play a critical role in the design and operation of our community clinic, providing direct care and supervising a team of healthcare professionals.

Key Responsibilities
  • Clinical Duties:
    • Conduct comprehensive assessments, diagnoses, and treatment plans for patients, providing health education and counseling as needed.
    • Manage patient care and treatment plans, ensuring accurate and up-to-date medical records.
    • Provide direct and indirect chronic disease care, screening, and health promotion, collaborating with physicians as necessary.
  • Administrative Duties:
    • Organize appointment schedules, manage referrals, and maintain up-to-date patient documentation.
    • Participate in program planning and development, ensuring alignment with our organizational goals.
  • Supervisory Duties:
    • Foster a positive and inclusive organizational culture, leading by example and promoting internal communication.
    • Monitor training and development, set performance goals, conduct reviews, and ensure adherence to relevant legislation.
  • Organizational Duties:
    • Participate in team activities, events, and committees, maintaining appropriate boundaries and attending required meetings and training sessions.
Qualifications
  • Education:
    • Current registration with the College of Nurses of Ontario as an RN (extended class).
    • Membership in RNAO and NPAO is an asset.
    • NP-Primary Health Care specialty certificate.
    • Master's degree and diabetic educator certification preferred.
  • Experience:
    • Proven experience in primary health care or community settings, working with individuals facing barriers to healthcare.
    • Leadership experience in team settings, with a focus on health promotion and illness prevention.
  • Skills:
    • Strong verbal and written communication, conflict resolution, and negotiation skills.
    • Ability to work independently and as part of a multidisciplinary team.
    • Knowledge of community resources and a commitment to an integrated model of practice.
    • Strong time management, prioritization, and innovation skills.
