Key Responsibilities- Clinical Duties:
- Conduct comprehensive assessments, diagnoses, and treatment plans for patients, providing health education and counseling as needed.
- Manage patient care and treatment plans, ensuring accurate and up-to-date medical records.
- Provide direct and indirect chronic disease care, screening, and health promotion, collaborating with physicians as necessary.
- Administrative Duties:
- Organize appointment schedules, manage referrals, and maintain up-to-date patient documentation.
- Participate in program planning and development, ensuring alignment with our organizational goals.
- Supervisory Duties:
- Foster a positive organizational culture, leading by example and supporting internal communication.
- Monitor training and development, set performance goals, conduct reviews, and ensure adherence to relevant legislation.
- Organizational Duties:
- Participate in team activities, events, and committees, maintaining appropriate boundaries and attending required meetings and training sessions.
- Education:
- Current registration with the College of Nurses of Ontario as an RN (extended class).
- Membership in RNAO and NPAO is an asset.
- NP-Primary Health Care specialty certificate.
- Master's degree and diabetic educator certification preferred.
- Experience:
- Proven experience in primary health care or community settings, working with individuals facing barriers to healthcare.
- Leadership experience in team settings, with a focus on health promotion and illness prevention.
- Skills:
- Strong verbal and written communication, conflict resolution, and negotiation skills.
- Ability to work independently and as part of a multidisciplinary team.
- Knowledge of community resources and a commitment to an integrated model of practice.
- Strong time management, prioritization, and innovation skills.
