About the Role

We are seeking a highly skilled and compassionate Indigenous Social Worker to join our Virtual Psychiatry Unit at Fraser Health. As a key member of our multi-disciplinary team, you will provide culturally appropriate counseling and intermediary services to First Nations, Métis, and Inuit clients.

Key Responsibilities
  • Provide social work services for clients, families, and healthcare professionals to enhance service delivery through consultation, development of care plans, and referrals to community resources.
  • Conduct client assessments and screenings, including evaluation of emotional, physical, mental, and spiritual health, as well as social determinants of health.
  • Participate in a multi-disciplinary team, bridging Indigenous Health and Mental Health and Substance Use, and referring Indigenous clients to appropriate services.
  • Communicate with community agencies and programs to ensure the best possible care for clients and families after discharge from hospital.
  • Provide education and promote health promotion initiatives to individuals, families, and Indigenous communities to increase knowledge, capacity, and resources for holistic wellness.
Requirements
  • Bachelor's degree in Social Work from an approved School of Social Work.
  • Current full registration with the BC College of Social Workers.
  • Two years' recent, related clinical social work experience in a hospital or community setting, including one year of experience working with Indigenous clients.
  • Lived experience and/or knowledge of Indigenous health approaches, protocol, cultural issues, and practices.
