Responsibilities- Direct Patient Care: Provide high-quality, patient-centered care to clients in their homes, focusing on their physical, emotional, and social needs.
- Documentation: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
- Ongoing Assessments: Conduct regular assessments to identify changes in patient condition and adjust care plans accordingly.
- Medication Administration: Prepare, administer, and document medications as prescribed by healthcare professionals.
- Skills and Treatments: Perform all skills, treatments, and procedures competently in accordance with the provincial scope of practice.
- Infection Control: Follow principles of infection control and Universal Precautions to ensure a safe environment for patients and staff.
- Certification and Licensure: Current certificate of competence and good standing as a Licensed Practical/Registered Practical Nurse.
- CPR Certification: Certified in CPR and able to respond to emergencies.
- Clinical Skills: Strong clinical skills and ability to work independently with minimal supervision.
- Customer Service: Excellent customer service skills and ability to communicate effectively with patients, families, and healthcare professionals.
