Your Opportunity:

Position Overview:

As a Clinical Coordinator for Donor-Recipient Programs at Alberta Health Services, you will assume a pivotal role in orchestrating the journey of patients and their families throughout the transplant continuum, from pre-transplant evaluations to post-operative care. You will collaborate closely with a multidisciplinary team to ensure the delivery of high-quality, personalized care that addresses all facets of the transplant experience, including specialized education for both patients and healthcare professionals attending the transplant clinic.

Key Responsibilities:

  • Facilitate seamless communication and coordination among patients, families, and healthcare providers.
  • Support the implementation of individualized care plans that reflect the unique needs of each patient.
  • Contribute to the development and delivery of educational programs for patients and staff.
  • Uphold the principles of safe, quality, and patient-centered care in all interactions.

Essential Qualifications:

Completion of an accredited nursing program is required. Candidates must be actively registered or eligible for registration with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification is mandatory.

Additional Qualifications:

A minimum of five years of experience in critical care and/or emergency nursing within the last seven years is essential, particularly with patients experiencing chronic illnesses and end-stage organ failure. Experience in case management within a healthcare setting is also required. Strong interpersonal, written, and verbal communication skills are crucial, along with demonstrated leadership, problem-solving, and decision-making abilities. Proficiency in Connect Care training and intermediate computer skills (Word, Excel, and PowerPoint) are necessary.

Preferred Qualifications:

Experience in providing quality, patient-centered care is highly valued. Previous involvement in the care of transplant patients is advantageous.
