Job Summary

Humber River Health is seeking a skilled Physician to join our team in the Post Acute and RCC Program. As a member of our Department of Medicine, you will provide comprehensive medical care to patients in this program.

Key Responsibilities

  • Provide medical care to patients in the Post Acute and RCC Program
  • Work collaboratively with Nurse Practitioners, nursing staff, and other healthcare professionals to deliver comprehensive care
  • Develop and implement treatment plans to address acute medical and aging conditions
  • Conduct assessments and manage complications to prevent or delay them
  • Restore optimal functional ability through an enhanced activation program

Requirements

  • Relevant certification from the Royal College of Physicians & Surgeons of Canada
  • Membership in the College of Physicians & Surgeons of Ontario
  • Experience in team-based care of older adults
  • Strong interpersonal skills
  • Additional training in Care of the Elderly is preferred
