Job Title: Mental Health Program Manager

Sienna Senior Living is seeking an exceptional Mental Health Program Manager to join our team. As a key member of our leadership team, you will play a pivotal role in developing, implementing, and sustaining care pathways focused on serving residents with mental health needs in our long-term care community.

Key Responsibilities:
  • Develop specific criteria and attributes to identify residents with mental health and clinical needs
  • Research, design, and implement all components of mental health programs
  • Provide consultation, assessment, development, implementation, and evaluation of individual plans of care
  • Create a measurement-based program plan
  • Design an approach for urgent and developing mental health needs
  • Conduct rounds to ensure plans of care are meeting the needs of the residents
Requirements:
  • Registered Nurse, Advanced Practice Nursing, or Regulated Health Professional in a relevant field
  • 5 years clinical experience involving clients with mental health and addiction issues
  • Design experience in developing programs using evidence-based practices
  • Experience in leading change, monitoring, and measuring outcomes, and the ability to provide advanced clinical care
  • Excellent interpersonal, communication, negotiation, teaching, and computer skills
