Join Vancouver Coastal Health (VCH) as a Geriatric Outreach Nurse

Vancouver Coastal Health is seeking a Registered Nurse or Registered Psychiatric Nurse to be part of our dedicated team as a Geriatric Outreach Nurse. This role is centered on providing care to older adults facing challenges such as dementia, significant psychiatric disorders, concurrent issues, and other age-related complications that may put them at risk due to a mix of psychiatric symptoms and complex medical or social factors. Responsibilities of the Geriatric Outreach Nurse include:

Facilitating integration and continuity across the healthcare spectrum: Conducting psychogeriatric evaluations Implementing crisis intervention strategies Coordinating case management Developing treatment plans Providing consultation and liaison services Leading individual and group therapy sessions Offering individual counseling, education, and support Collaborating with general practitioners, caregivers, and family members while working alongside staff in various care settings. Organizing community resources and delivering educational initiatives as necessary. Building and maintaining relationships with clients, their families, and colleagues to ensure the delivery of appropriate treatment services grounded in best practices. Sharing expertise with colleagues through consultations, training sessions, and presentations. Serving as a clinical resource for nursing staff and other healthcare professionals in both hospital and community settings. Providing direct care and outreach services as needed.

Required Qualifications

Education & Experience

Current registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). A minimum of three (3) years of recent experience in geriatric mental health and addictions or an equivalent combination of education and experience. A valid Driver's License and access to a vehicle are mandatory.

Knowledge, Skills & Abilities

Comprehensive understanding of gerontology and geriatric psychiatric nursing standards as outlined by the Canadian Gerontological Nursing Association. Familiarity with concurrent disorders. Expertise in assessing and treating psychiatric patients, including knowledge of geriatric psychopathology such as depression, delusions, delirium, and dementia. Awareness of issues related to aging, chronic illness, and psychosocial factors including caregiver dynamics and communication patterns. Knowledge of detoxification and withdrawal management, as well as harm reduction strategies for older adults with substance use disorders. Understanding of psychopharmacology, including indications and side effects. Proficiency in crisis intervention and familiarity with psychiatric services and community resources. Demonstrated ability to provide counseling, psycho-education, and consultation services. Insight into the roles of hospital and community geriatric services in client care and the ability to act as a liaison between these services. Knowledge of family dynamics and end-of-life issues. Competence in performing relevant assessments using validated tools. Proven ability to create holistic care plans that encompass physical, behavioral, social, and emotional factors. Strong interpersonal skills to build trust and ethical relationships with clients, families, and healthcare professionals. Effective communication skills, both verbal and written, with clients, families, colleagues, and other healthcare staff. Ability to work independently while also being a collaborative team member, leader, and facilitator. Commitment to client-centered care that respects diverse cultures and advocates for social justice. Knowledge of relevant legislation and government policies regarding client care, including abuse and neglect laws. Adaptability to new or unexpected situations and effective conflict resolution skills. Capability to apply theoretical knowledge to case management. Skills in developing and delivering educational programs and materials for staff, patients, and families based on established educational theories and best practices. Basic understanding of research ethics and methodologies. Strong organizational skills to plan, schedule, and prioritize work effectively. Proficiency in CPR techniques and nursing treatment equipment. Physical capability to perform the duties associated with this position.
