Vancouver Coastal Health is seeking a Registered Nurse or Registered Psychiatric Nurse to fill the role of Older Adult Outreach Nurse. This position involves working with older adults who are experiencing dementia or have major psychiatric disorders, concurrent disorders, and/or problems associated with aging.
Key Responsibilities- Perform psychogeriatric assessments and provide crisis intervention services.
- Coordinate case management and develop treatment plans in collaboration with healthcare teams.
- Provide consultation and liaison services to healthcare providers and community resources.
- Facilitate individual and group sessions to promote client-focused care.
- Consult with general practitioners, caregivers, and family members to provide holistic care.
- Coordinate community resources and provide educational programs as required.
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Three years of recent experience working in geriatric mental health and addictions or an equivalent combination of education and experience.
- A valid driver's license and use of a vehicle are required.
