Job Summary

We are seeking a highly skilled and compassionate Outreach Worker Forensic Psychiatric Services Specialist to join our team at PHSA. As a key member of our Forensic Assertive Community Treatment Team, you will play a vital role in providing support and services to individuals with complex mental health and substance use challenges.

Key Responsibilities

  • Conduct thorough assessments and screenings of clients to identify their health needs and develop personalized care plans.
  • Establish and maintain relationships with clients, their families, and community resources to ensure seamless care coordination.
  • Provide emotional support and guidance to clients, promoting their independence and self-management skills.
  • Collaborate with multidisciplinary teams to develop and implement treatment plans, ensuring a comprehensive and coordinated approach to care.
  • Act as a liaison between clients, families, and community agencies, facilitating access to resources and services.
  • Participate in ongoing education and training to enhance knowledge and skills in areas such as Indigenous Cultural Safety, trauma-informed care, and substance use treatment.

Requirements

  • Graduation from a recognized Social Services Worker certificate program or equivalent combination of education and experience.
  • Valid Class 5 vehicle license.
  • Strong understanding of Indigenous Cultural Safety principles and commitment to reconciliation.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines.
