Job Summary:

Bonaventure Support Services is seeking a skilled Community Housing Health Care Assistant to join our team in the Parksville area. As a key member of our team, you will provide daily living support to individuals with diverse abilities, promoting their independence, dignity, and well-being.

Responsibilities:

  • Provide personal care and support to individuals with diverse abilities, including assistance with daily living activities, medication management, and behavioral support.
  • Collaborate with healthcare professionals, social workers, and other stakeholders to ensure comprehensive care and support.
  • Develop and maintain person-centered plans to promote independence, dignity, and well-being.
  • Administer medications, monitor health status, and maintain accurate records.
  • Provide crisis intervention and risk management strategies to ensure a safe and supportive environment.
  • Participate in team meetings, training, and professional development to enhance skills and knowledge.

Requirements:

  • Valid BC Class 4 Driver's License or willingness to obtain within 3 months.
  • RCMP Criminal Record Check (Schedule A) and MCFD Hub Screening.
  • First Aid with CPR Certificate and Food Safe Level 1 Certificate.
  • Immunization Records or Medical Exemption.
  • Physician's Declaration and TB Screen.
  • Three professional references, including a most recent employer.
