Community Housing Support Worker

Job Summary:
Bonaventure Support Services is seeking a skilled Community Housing Health Care Assistant to join our team. As a key member of our support team, you will work collaboratively with individuals who are medically fragile and have limited mobility in the Parksville area. Your primary responsibility will be to provide daily living support, including personal care, health, and peri-care, while promoting dignity, independence, and individuality.
Responsibilities:
- Provide daily living support, including personal care, health, and peri-care, to individuals with diverse abilities.
- Assist with activities of daily living, such as bathing, dressing, and grooming.
- Administer medications, monitor, and maintain accurate records, following established protocols and guidelines.
- Develop and maintain person-centered plans (PCPs) to meet the unique needs of each individual.
- Collaborate with the Program Manager and other support team members to ensure a comprehensive and coordinated approach to care.
- Participate in crisis intervention and risk management strategies to ensure the safety and well-being of individuals and staff.
- Provide emotional support and promote a positive and respectful environment.
- Maintain accurate records and reports, as required.
Qualifications:
- Preferred formal education (Diploma or Degree, CSW, RCA, SIVA, etc.) and/or work experience with persons with diverse abilities and/or other relevant experience.
- Excellent interpersonal behavior and communication skills.
- The desire and ability to work with persons with diverse abilities and demonstrate personal suitability to promote dignity, independence, and individuality.
- Ability to work independently in a high-stress environment.
- Must be in good health with the physical ability to carry out the duties of the position and shift responsibilities.
Required Documentation:
- Valid BC Class 4 Driver's License - or willingness to obtain one within 3 months of employment.
- RCMP Criminal Record Check (Schedule A - Employer will file).
- MCFD Hub Screening (Employer will file).
- First Aid with CPR Certificate.
- Food Safe Level 1 Certificate.
- Immunization Records or Medical Exemption.
- Signed Bonaventure Hire Package.
- 3 References - (One must be a most recent employer, second and third must be able to speak to your ability to work with vulnerable youths).
- Physician's Declaration.
- TB Screen.
