Job Title: Physiotherapist - Community Care Specialist

Job Summary:

Bayshore HealthCare is seeking a skilled Physiotherapist to join our team in providing community-based physiotherapy services. As a Physiotherapist - Community Care Specialist, you will be responsible for assessing, treating, and educating clients in their homes, promoting independence and improving their quality of life.

Key Responsibilities:

  • Conduct thorough assessments of clients to identify their physical and functional needs
  • Develop and implement individualized treatment plans to address client goals and objectives
  • Provide evidence-based physiotherapy interventions to improve client function and mobility
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination

Requirements:

  • Master's degree in Physiotherapy or recognized equivalent
  • Registration with the College of Physiotherapists of Ontario
  • Minimum 3 years of experience in physiotherapy, preferably in home care or community settings
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
