Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Home Care Specialist. As a key member of our community nursing team, you will provide exceptional nursing services to patients in their homes, clinics, and other community settings.

Key Responsibilities
  • Develop and implement effective care plans in collaboration with patients, families, and healthcare teams
  • Provide high-quality nursing services, including wound care, medication management, and vital sign monitoring
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
  • Contribute to the development and implementation of new care programs and services
Requirements
  • Current registration with the College of Nurses of Ontario
  • Valid driver's license and reliable vehicle
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
