Job Description

Job Summary: We are seeking a skilled Assessor Coordinator Degree to join our team at Health Careers in Saskatchewan. As a key member of our healthcare team, you will be responsible for providing case management services, including assessment, planning, monitoring, and evaluation.

Key Responsibilities:

  • Provide case management services to clients, including assessment, planning, monitoring, and evaluation.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with healthcare professionals and community resources to ensure comprehensive care.
  • Conduct thorough assessments to identify client needs and develop effective care plans.
  • Monitor client progress and make adjustments to care plans as needed.

Requirements:

  • Bachelor of Social Work (BSW) or Bachelor Degree in a health-related field.
  • Registered/eligible with the Saskatchewan Association of Social Workers (SASW).
  • Knowledge of community resources and applicable programming.
  • Excellent communication and interpersonal skills.

Preferred Qualifications:

  • Experience working in a healthcare setting.
  • Knowledge of healthcare policies and procedures.
  • Ability to work independently and as part of a team.
