Job Title: Assessor/Coordinator

**Job Summary:**

We are seeking a highly skilled Assessor/Coordinator to join our team at Saskatoon Home Care Office. As an Assessor/Coordinator, you will provide case management services, including assessment, planning, monitoring, and evaluation, to clients/patients accessing community-based services within SHA.

**Key Responsibilities:**

  • Provide case management services, including assessment, planning, monitoring, and evaluation
  • Coordinate access to community-based services within SHA
  • Develop and implement individualized care plans
  • Monitor and evaluate client/patient progress

**Requirements:**

  • Bachelor of Social Work (BSW) or Bachelor Degree in a health-related field
  • Assessment knowledge and skills
  • Knowledge of SHA, community resources, and applicable programming
  • Registered/eligible with the Saskatchewan Association of Social Workers (SASW)

**Preferred Qualifications:**

  • HSAS membership
  • Registration/eligibility for registration with the appropriate licensing body

**What We Offer:**

  • Competitive salary range: $36.721 to $44.902
  • Full-time regular position
  • 14 shifts of 8 hours per 3 weeks
