Job Summary
Vancouver Coastal Health is seeking a skilled Social Worker to join their team at Lions Gate Hospital in North Vancouver, BC. As a Social Worker with VCH, you will provide clinical and consultative social work services to clients and families, conducting social assessments, providing counseling services, and maintaining records.
Key Responsibilities
- Provide clinical and consultative social work services to clients and families
- Conduct social assessments and provide counseling services
- Maintain accurate and up-to-date records
- Attend interdisciplinary clinical/case conferences
- Identify and facilitate access to community resources
Qualifications
Education and Experience
- Bachelor of Social Work from an accredited School of Social Work
- One year of post-Bachelor of Social Work experience or successful completion of a practicum in a healthcare setting
Knowledge, Skills, and Abilities
- Basic knowledge of systems thinking and social work process
- Basic knowledge of counseling skills and other health disciplines
- Ability to communicate effectively with clients, families, and healthcare teams
- Ability to work collaboratively as a member of an interdisciplinary team
