Vancouver Coastal Health is seeking a skilled Social Worker to join our team in Vancouver, BC. As a Social Worker with our organization, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.
Key Responsibilities- Provide clinical and consultative social work services to clients and families.
- Conduct social assessments and develop comprehensive treatment plans.
- Provide counseling services and support to clients and their families.
- Maintain accurate and up-to-date client records.
- Collaborate with interdisciplinary teams to provide comprehensive care.
- Identify and connect clients with community resources and services.
- Current full registration with the British Columbia College of Social Workers.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two (2) years' recent related experience working with marginalized clients with multiple health issues.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
