About the Role:
Vancouver Coastal Health is seeking a skilled Social Worker to join our Overdose Outreach team in Vancouver, BC. As a Social Worker with our organization, you will play a vital role in providing clinical and consultative social work services to clients and families.
Key Responsibilities:
- Provide social assessments and counseling services to clients and families.
- Develop and implement individualized care plans to address clients' needs.
- Collaborate with interdisciplinary teams to provide comprehensive care.
- Identify and connect clients with community resources and services.
- Maintain accurate and confidential client records.
Requirements:
- Current full registration with the British Columbia College of Social Workers.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two years' recent experience working with marginalized clients with multiple health issues in community-based or Indigenous-led organizations.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
