Job Overview

As a pivotal member of the healthcare team, the Care Management Leader is responsible for ensuring that patient care is delivered in accordance with the highest standards of professional practice. Reporting directly to the Manager of Care Management & System Navigation, this role emphasizes a collaborative approach within a client and family-centered care framework, aligned with the mission and values of Providence Health Care.

Key Responsibilities

  1. Facilitates effective patient care from admission through discharge, coordinating with the interprofessional team to develop and implement sustainable discharge plans.
  2. Conducts thorough nursing assessments and guides the interprofessional team on the necessary interventions and resources to optimize patient outcomes.
  3. Accountable for the formulation and efficacy of policies and procedures that support robust care management and discharge planning.
  4. Engages in ongoing education and participates in various committees and corporate initiatives to enhance care delivery.
  5. Utilizes data to assess practice effectiveness, focusing on patient-centered care and quality improvement.

Qualifications

Education: Must possess current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). A minimum of three years of relevant experience, including at least one year in community care and discharge planning, is required.

Skills and Abilities: The ideal candidate will demonstrate:

  • Extensive knowledge of nursing theory and practice within a patient-centered care model.
  • Familiarity with BCCNM standards and evidence-based nursing practices.
  • Strong analytical skills to assess patient needs and develop appropriate care plans.
  • Effective communication skills to collaborate with patients, families, and healthcare professionals.
  • Proficiency in utilizing computerized client care information systems.

Role Expectations

The Care Management Leader will:

  1. Lead care coordination and discharge planning efforts, ensuring timely and safe transitions for patients.
  2. Monitor patient progress and adjust care plans as necessary, involving patients and families in goal-setting.
  3. Develop and recommend policies to enhance care management processes.
  4. Act as a resource for staff and families, providing guidance on care management and discharge planning.
  5. Maintain accurate patient records and promote best practices within the organization.
