Job SummaryWe are seeking a highly skilled and compassionate RN Hospice Palliative Care Specialist to join our interdisciplinary team at St. John's Hospice. As a key member of our team, you will provide holistic care to patients and their families, working collaboratively with our interdisciplinary team to deliver comprehensive and compassionate care.
Key Responsibilities- Assess patient needs and develop individualized care plans
- Provide direct nursing interventions for management of crisis and life-threatening situations
- Monitor patient progress, evaluate outcomes, and modify care plans as needed
- Document patient information, including assessments, care plans, and progress notes
- Collaborate with interdisciplinary team members to ensure comprehensive care
- Participate in nursing orientation and share clinical knowledge with other nursing staff
- Participate in quality improvement activities and contribute to the development of best practices
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
- Certification in Hospice/Palliative Care
- One year's recent, related palliative nursing care experience in an acute care facility or an equivalent combination of education, training, and experience
- Ability to communicate effectively, both verbally and in writing
- Ability to work as a member of an interdisciplinary team
- Ability to apply decision-making and analytical skills
- Ability to deal with others effectively
- Ability to role model conflict resolution strategies
- Physical ability to carry out the duties of the position
