Job Summary

We are seeking a highly skilled and compassionate RN Hospice Palliative Care Specialist to join our interdisciplinary team at St. John's Hospice. As a key member of our team, you will provide holistic care to patients and their families, working collaboratively with our interdisciplinary team to deliver comprehensive and compassionate care.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Provide direct nursing interventions for management of crisis and life-threatening situations
  • Monitor patient progress, evaluate outcomes, and modify care plans as needed
  • Document patient information, including assessments, care plans, and progress notes
  • Collaborate with interdisciplinary team members to ensure comprehensive care
  • Participate in nursing orientation and share clinical knowledge with other nursing staff
  • Participate in quality improvement activities and contribute to the development of best practices
Requirements
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
  • Certification in Hospice/Palliative Care
  • One year's recent, related palliative nursing care experience in an acute care facility or an equivalent combination of education, training, and experience
  • Ability to communicate effectively, both verbally and in writing
  • Ability to work as a member of an interdisciplinary team
  • Ability to apply decision-making and analytical skills
  • Ability to deal with others effectively
  • Ability to role model conflict resolution strategies
  • Physical ability to carry out the duties of the position
