Community Integration Specialist

The Community Integration Specialist is responsible for assessing, supporting, and guiding clients at the Restigouche Hospital Center in fostering their independence. This role aligns with the interdisciplinary intervention strategy aimed at facilitating their return to the community through the execution of treatment and rehabilitation initiatives. The position encompasses a variety of intervention techniques, including one-on-one client sessions, support for therapeutic groups led by nursing staff and other professionals, as well as the organization of educational workshops.
KEY RESPONSIBILITIES:
- Evaluate and assist clients in their journey towards autonomy;
- Conduct individual meetings and facilitate group therapy sessions;
- Implement educational programs that promote client engagement;
- Ensure continuity of care through organized service delivery;
- Adhere to clinical principles focused on recovery, risk assessment, and responsiveness;
- Follow the ethical guidelines and policies set forth by the Vitalité Health Network.
QUALIFICATIONS:
- High school diploma or equivalent, supplemented by a community integration program or course lasting between six months to one year;
- Current certification in Basic Life Support (BLS);
- Training in Non-Violent Crisis Intervention (CPI) or willingness to participate in scheduled training;
- Training in Suicide Prevention and Management or commitment to undergo necessary training;
- Valid driver's license in accordance with New Brunswick Motor Vehicle Act;
- Strong judgment and organizational capabilities;
- Proficiency in utilizing diverse intervention strategies creatively;
- Ability to work autonomously as well as collaboratively;
- Physical capability to perform designated tasks;
- Positive prior work history regarding performance and attendance;
- Commitment to professional ethics and organizational values;
- Respect for confidentiality protocols within the Vitalité Health Network;
- The selection process may include assessments through oral, written, or practical evaluations;
