Job Summary

We are seeking a skilled Licensed Practical Nurse to join our Renal Hemodialysis team at Vancouver Coastal Health. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our multidisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Provide direct patient care to clients with renal failure, including assessing, planning, implementing, and evaluating interventions.
  • Work closely with the Registered Nurse to develop and implement individualized care plans that meet the unique needs of each patient.
  • Participate in care planning, client advocacy, and education to promote health and wellness.
  • Collaborate with other healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Document patient care and respond to incidents in accordance with established policies and procedures.
Qualifications
  • Graduation from a recognized program for Practical Nurses and two years of recent related experience working in an acute care environment.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives.
  • Demonstrated ability in nursing practice related to designated client group – Renal Hemodialysis.
