Job Summary

We are seeking a highly skilled Registered Practical Nurse to join our team at North York General Hospital. As a Registered Practical Nurse, you will play a vital role in providing excellent patient and family-centered care in our Reactivation Care Centre.

Key Responsibilities
  • Provide comprehensive nursing care to patients, including assessment, education, prevention, maintenance, and restoration of patient health.
  • Collaborate with an interdisciplinary team of healthcare providers to ensure seamless care delivery.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Conduct thorough health assessments and provide accurate documentation of patient information.
  • Communicate effectively with patients, families, and healthcare teams to ensure optimal care outcomes.
  • Participate in quality improvement initiatives to enhance patient care and safety.
Requirements
  • Registered Practical Nursing diploma from a recognized Community College.
  • Registered and in good standing with the College of Nurses of Ontario.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • Minimum 2 years of recent Medical/Surgical and/or clinical nursing experience working with older adult, dementia, and cognitively impaired populations.
  • Recent health assessment course.
  • Bilingualism in French/English an asset.
