The Healthcare Social Worker is a key member of the interdisciplinary healthcare team, providing collaborative and holistic care on the primary to quaternary care continuum. This role focuses on the relationships between individuals and their environments, which are integral to psychosocial, emotional, and spiritual well-being. The social determinants of health are embedded in social work theory and practice, and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention, and evaluation.
About YouWe are seeking a skilled and experienced Social Worker to join our team. The ideal candidate will have:
- A Master of Social Work degree from an accredited university, or be enrolled in a master's program, or have a Bachelor of Social Work degree with relevant experience.
- Registration or eligibility for registration with the Nova Scotia College of Social Workers (NSCSW).
- A minimum of two years of clinical experience, with evidence of extensive knowledge of community resources.
- Ability to work independently with a multidisciplinary team, as well as with hospital and community-based services/agencies.
- Competencies in other languages are an asset, with French being preferred.

Key ResponsibilitiesThe Healthcare Social Worker will be responsible for:
- Providing collaborative and holistic care to patients and families.
- Assessing and addressing the social determinants of health.
- Developing and implementing care plans for hospital discharge and safe transition back to the community or institutional setting.
- Coordinating with multidisciplinary teams to ensure comprehensive care.
- Documenting and evaluating patient care.
Nova Scotia Health Authority offers a competitive compensation package, including:
- A hourly rate of $49.05.
