Role Overview
As a Paediatric Liaison Specialist, you will play a crucial role in coordinating and facilitating the smooth transition of young patients between various healthcare settings. Your expertise will be instrumental in ensuring continuity of care and optimal outcomes for children under our care.
Key Responsibilities
- Provide expert clinical advice and guidance to families, healthcare professionals, and other stakeholders involved in a child's care journey.
- Collaborate effectively with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each child.
- Conduct thorough assessments of children's health status, identifying potential risks and areas requiring intervention.
- Advocate for the best interests of children and their families, ensuring their voices are heard and respected throughout the healthcare process.
- Stay abreast of current best practices and advancements in paediatric care, incorporating evidence-based knowledge into clinical decision-making.
Essential Skills and Qualifications
- Registered Health Visitor or School Nurse with a minimum of 1 year post-registration experience.
