About This Opportunity

The Nova Scotia Health Authority's innovative Care by Design initiative offers residents of long-term care facilities access to exceptional and consistent primary health services. This program aims to enhance the quality of life for our most vulnerable seniors while ensuring an appropriate healthcare approach tailored to their specific needs. Care by Design emphasizes comprehensive, collaborative, and resident-centered care. As a physician within this program, you will collaborate with a dedicated team of family physicians focused on improving the continuity and quality of care for residents in long-term care settings. This includes ensuring the safety of medication regimens, minimizing unnecessary transfers to emergency departments, and empowering residents to actively participate in their care planning.

Physicians are assigned to a specific geographical network, which facilitates minimal travel and enhances communication with facilities and residents. These networks provide robust on-call services, efficient training, and valuable quality improvement opportunities. Physicians are designated to one floor/wing/unit in a facility and are expected to visit at least once a week at a scheduled time. After-hours on-call support is provided by a group of physicians and is centrally managed, typically occurring once every two months. Physicians are compensated on a fee-for-service basis under NS MSI, with additional stipends for on-call shifts, and there are no overhead costs associated with participation in Care by Design. Discussions regarding the Longitudinal Family Medicine model can also be arranged.

Responsibilities
  • Provide comprehensive primary healthcare services to patients of all ages, with a particular emphasis on geriatric care.
  • Conduct rounds on patients at the long-term care facility at least once a week and be available for on-call duties during standard business hours.
  • Collaborate with other healthcare providers in the geographical network to ensure evening and weekend coverage.
  • Utilize an electronic medical record (EMR) system to document all relevant patient information in a provincially approved EMR accessible to all collaborating team members.
Expected Hours
  • Permanent or locum, part-time positions available.
  • Minimum requirement to conduct patient rounds once a week.
  • Shared evening and weekend on-call responsibilities.
Qualifications
  • All applicants must be eligible for licensure with the College of Physicians and Surgeons of Nova Scotia (CPSNS).
  • All specialists must be eligible for certification with the Royal College of Physicians and Surgeons of Canada (RCPSC).
  • Membership with the Canadian Medical Protective Association (CMPA) is required.
