Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Fraser Health. As a Community Health Nurse, you will play a vital role in providing high-quality nursing care to our clients in a community health setting.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of clients' physical, emotional, and social needs, and develop personalized care plans to address their unique requirements.
  • Client Care: Provide direct care to clients, including administering medications, assisting with activities of daily living, and promoting independence and self-care.
  • Collaboration and Communication: Work effectively with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure seamless care coordination and communication.
  • Education and Support: Educate clients and their families on health management, disease prevention, and wellness strategies, and provide emotional support and guidance as needed.
  • Quality Improvement: Participate in quality improvement initiatives to enhance the delivery of care and services, and contribute to the development of policies and procedures to ensure best practices.
Requirements
  • Education: Graduation from an approved program of Practical Nursing, or an equivalent combination of education, training, and experience.
  • Registration: Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Certifications: Current certificates in CPR Level C or equivalent and First Aid.
  • Skills and Abilities: Excellent communication and interpersonal skills, physical ability to perform the duties of the position, and ability to work independently and as part of a team.
