About the Role

VON Canada is seeking a Licensed Practical Nurse to join our team in providing high-quality care to clients in their homes and communities.

Key Responsibilities
  • Provide nursing care to clients in their homes and communities, as per their individualized care plans.
  • Conduct thorough assessments of clients' physical and emotional needs, and develop and implement care plans to meet those needs.
  • Administer medications and treatments as prescribed by physicians, and monitor clients' responses to care.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document client care and progress in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current licensure as a Licensed Practical Nurse (LPN) in the province of employment.
  • Experience working in home care or visiting care agency settings, an asset.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality care.
  • Valid driver's license and reliable vehicle, as travel to clients' homes is required.
