Blue Door Shelters is a not-for-profit charitable organization governed by a volunteer Board of Directors. Since its inception, we have played a vital role in our community by providing essential emergency shelter services.

Our mission is to assist individuals at risk of or experiencing homelessness in achieving and maintaining affordable housing.

Reporting to the Program Manager, the Medical Services Supervisor is a qualified social worker or equivalent professional responsible for case management, coordinating and facilitating services for individuals and families facing homelessness.

The Medical Services Supervisor will ensure that clients with intricate needs receive the comprehensive support necessary for successful reintegration into the community and provide ongoing assistance to help them achieve their objectives.
Review completed client service packages and identify clients with more complex requirements.
Assess strengths, risks, needs, and goals of clients to develop tailored case management plans.
Guide team members in supporting client objectives and plans.
Assist the Manager in organizing staff meetings to accommodate all shifts.
Build, nurture, and enhance collaborative relationships with community service providers, including expanding the existing network of community resources.

Refer clients to suitable services, advocate as necessary, and follow up to ensure successful referrals and connections.

Facilitate comprehensive support for clients.

Oversee the discharge planning process for clients to ensure that support and services are established to promote success in maintaining housing options.

Complete necessary documentation, including reports and case notes, and gather data as required.
Adhere to the mission, vision, policies, and procedures of the organization.
Participate in internal staff meetings and training sessions as required.
Utilize effective conflict resolution skills in accordance with Crisis Prevention Intervention (CPI) training.
Foster positive and appropriate relationships with clients, colleagues, community partners, and volunteers.
Meet regularly with the manager and contribute to the annual performance evaluation process.
All employees are expected to understand and comply with health and safety policies and procedures.
Minimum Bachelor of Social Work Degree or an equivalent combination of education and experience at the Bachelor level.
A minimum of three years of experience in a social service environment.
Knowledge of community resources available in the region.
Experience working with diverse, multicultural client populations, including those facing homelessness, mental health challenges, and addiction issues.
Demonstrated proficiency in computer skills, including Microsoft Office.
Crisis prevention training is considered an asset.
A valid driver's license, reliable vehicle, and insurance for work-related travel.
