Job Summary

We are seeking a highly skilled and experienced Mental Health Program Manager to join our team at Sienna Senior Living. As a key member of our leadership team, you will be responsible for developing, implementing, and sustaining care pathways focused on serving residents with mental health needs in our long-term care community.

Key Responsibilities
  • Program Development & Implementation:
    • Develop specific criteria and attributes that would identify residents with mental health and clinical needs
    • Research, design, and implement all components of mental health programs
  • Provide Consultation, Assessment, Development, Implementation, and Evaluation of Individual Plans of Care:
    • Create a measurement-based program plan
    • Design an approach for urgent and developing mental health needs
  • Conduct Rounds to Ensure Plans of Care are Meeting the Needs of Residents:
    • Collaborate with leadership and team members to ensure programs are designed and implemented in alignment with Sienna's mission, vision, and values
Requirements
  • You are a Registered Nurse, an Advanced Practice Nurse, or a Regulated Health Professional in a relevant field
  • You are in good standing with your associated regulatory College
  • You require 5 years of clinical experience involving clients with mental health and addiction issues
  • You have design experience in developing programs using evidence-based practices
  • You have experience in leading change, monitoring, and measuring outcomes, and the ability to provide advanced clinical care
  • You have excellent interpersonal, communication, negotiation, teaching, and computer skills
