About the Opportunity

The Care Coordinator is a key member of the healthcare team responsible for assessing, planning, and coordinating care for clients referred to Continuing Care. This role involves working with clients, families, and healthcare professionals to identify goals, unmet needs, and resource requirements.

The Care Coordinator will work with interprofessional teams to facilitate hospital discharge and return to the community, authorizing services and making referrals to other professional and volunteer resources as needed. They will also maintain ongoing communication with clients, family, caregivers, care providers, and healthcare teams within the client's identified circle of care.

The philosophy of the Care Coordinator is grounded in 'Home First': maximizing available services and resources to support clients and families in the home/community. This role requires strong assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills.

Key Responsibilities:

  • Assess client needs and develop care plans
  • Authorize services and make referrals to other resources
  • Work with interprofessional teams to facilitate hospital discharge
  • Maintain ongoing communication with clients and healthcare teams
  • Advocate for clients and families to access available services and resources

Requirements:

  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in community-based care management
  • Demonstrated knowledge and/or experience in a community setting
  • Knowledge and/or experience in dedicated case management or discharge planning specific to caring for the unhoused an asset
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and co-operatively in a busy multidisciplinary situation
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle required
  • Competencies in other languages preferred; French an asset
