In alignment with Northern Health's vision for an integrated service system, the Primary Care Nurse (PCN) operates under the standards of professional practice and ethical guidelines set forth by the BC College of Nurses and Midwives (BCCNM).
As a vital member of the interprofessional team, the PCN employs best practices to deliver safe, competent, and ethical care to individuals and families across various settings, including physician offices, homes, schools, and community environments.
The PCN is responsible for clinical follow-ups in accordance with the individual's Primary Care Plan and personal care goals, collaborating with the interprofessional team to develop comprehensive care plans.
Primary Care Networks consist of partnerships among family practice divisions, regional health authorities, the First Nations Health Authority (FNHA), and local community stakeholders, all striving to enhance the primary and community care system for improved patient and provider experiences.
Work Schedule: Monday to Friday, Day shifts from 09:00 to 17:00. On-call duties are required for this role.
Nurses will rotate through Emergency and Community Services and participate in an on-call schedule for after-hours emergencies.

Qualifications
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years of recent, relevant clinical nursing experience in primary care and/or chronic disease management, including experience in patient education.
- Current valid Driver's License.
- Assessment and Intervention - Proven ability to conduct initial and ongoing client assessments and deliver nursing care through prescribed interventions.
- Communication - Effective communication skills with clients, families, the public, and interdisciplinary team members using various communication methods.
- Critical Thinking - Ability to analyze and evaluate data from multiple sources to solve problems effectively.
- Person and Family Centered Practice - Commitment to promoting person-focused care that respects diverse cultures and client advocacy.
- Teaching - Knowledge of adult learning principles to educate clients and families on health care topics.
