Primary Care Nurse

We are seeking a highly skilled Registered Nurse to join our team in Primary Care. The successful candidate will provide compassionate patient care and work collaboratively with our interprofessional team to deliver high-quality services to our patients.
Key Responsibilities- Provide clinical follow-up according to the patient's primary Care Plan and personal goals of care
- Develop and implement plans of care collaboratively with the interprofessional team
- Provide direct clinical care and services to individuals, groups, and communities
- Work across Emergency and Community Services, as well as rotate through an on-call schedule for after-hours emergencies
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
- Current valid Driver's License
