Primary Care Nurse

We are seeking a highly skilled and dedicated Primary Care Nurse to join our team at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and contributing to the development of our primary care services.
Job Summary:The Primary Care Nurse will work collaboratively with our interprofessional team to provide comprehensive care to patients in our primary care settings. This includes assessing patient needs, developing and implementing care plans, and providing education and support to patients and their families.
Responsibilities:- Assess patient needs and develop individualized care plans
- Provide direct patient care, including medication management and wound care
- Collaborate with the interprofessional team to coordinate patient care
- Provide education and support to patients and their families
- Participate in quality improvement initiatives to enhance patient care
- Current registration with the BC College of Nurses and Midwives
- Three years of recent experience in primary care nursing and/or chronic disease management
- Valid driver's license
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
