We are seeking a highly skilled and dedicated Primary Care Nurse to join our team at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and contributing to the development of our primary care services.
About the Role:- Provide comprehensive nursing care to patients in primary care settings, including clinics, hospitals, and community health centers.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Conduct assessments, diagnose, and treat patients with a range of health conditions, including chronic diseases and mental health issues.
- Collaborate with healthcare professionals to provide education and support to patients and families.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
