Position Overview

Vancouver Coastal Health (VCH) is seeking a Lead Coordinator for Community Programs.

The organization is in search of a dedicated professional to oversee the Palliative Care Program team.

Key Responsibilities:

  1. Report to the assigned Community Manager and take responsibility for the provision and integration of specific community programs within the Vancouver Coastal Health (VCH) Continuum of Care, which includes planning, developing, implementing, delivering, and assessing community program services.
  2. Collaborate with VCH acute and community personnel, physicians, community organizations, and other services to ensure that programs are effectively integrated along the continuum of care and executed within available resources.
  3. Encourage and facilitate community and stakeholder involvement in formulating policies and innovative solutions to health challenges related to health planning, program execution, and evaluation.
  4. Supervise the daily operations of designated programs, manage and lead interdisciplinary teams, participate in and/or lead committees and task groups, and work with colleagues, internal stakeholders, and researchers as necessary to achieve program objectives.
Required Qualifications

Education & Experience:

  1. A Bachelor's Degree (Master's preferred) in a relevant healthcare field.
  2. A minimum of five (5) to seven (7) years of recent, related clinical or professional experience, including direct supervisory experience or an equivalent combination of education, training, and experience.

Skills & Competencies:

  1. Proven experience in effectively developing and executing new programs and initiatives.
  2. Comprehensive understanding of current community health program theory and practice, along with the skillful application and development of relevant departmental policies and procedures, as well as municipal and provincial legislative requirements related to service implementation.
  3. Knowledge of a wide range of cross-cultural and diversity issues, with a strong understanding of the social determinants of health in a healthcare context. Experience in population health, chronic disease prevention, and primary care in community settings is essential.
  4. Ability to engage internal and external stakeholders to develop, manage, and evaluate clinical services, considering client needs and service delivery interrelationships within a complex environment.
  5. Demonstrated leadership skills to foster collaborative relationships that promote cooperative goal achievement and contribute to a culture of mutual trust and respect.
  6. Strong analytical skills to effectively review current processes, identify areas for improvement, write reports, and analyze statistical data.
  7. Ability to develop and monitor program budgets, track variances, and secure additional funding from various sources.
  8. Utilizes sound business judgment to manage fiscal resources responsibly and in alignment with the organization's overall goals.
  9. Demonstrated leadership, facilitation, collaboration, and negotiation skills for effective issue resolution.
  10. Applies project management methodologies in designing and developing various frameworks, tools, and techniques.
  11. Strong systems-thinking, critical analysis, problem-solving, and decision-making abilities.
  12. Excellent communication skills, both verbal and written, with the ability to engage effectively with individuals at all organizational levels.
  13. Commitment to staying informed about current legislation, regulations, industry trends, technology, and protocols, sharing knowledge, and supporting staff in integrating evidence-based practices in quality care delivery.
  14. Physical capability to perform the duties of the position.
