Job Summary:

Trillium Health Partners is seeking a highly organized and detail-oriented Unit Coordination Assistant to provide administrative support for patient care services. The successful candidate will be responsible for coordinating patient information, facilitating communication, and ensuring a smooth flow of information for effective patient care.

Key Responsibilities:

  • Coordinate and manage patient information, ensuring accuracy and timeliness.
  • Facilitate communication between patients, families, and healthcare teams.
  • Provide administrative support for patient care services, including data entry and record-keeping.
  • Assist with scheduling and coordination of patient appointments and tests.
  • Maintain patient charts and records, ensuring confidentiality and accuracy.
  • Provide excellent customer service to patients, families, and healthcare teams.
  • Collaborate with healthcare teams to ensure seamless patient care.
  • Perform other administrative tasks as required.

Requirements:

  • High school diploma or equivalent required; post-secondary education in healthcare administration or a related field preferred.
  • 1-2 years of experience in an administrative role, preferably in a healthcare setting.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Proficiency in basic computer applications and familiarity with Epic.
