We are currently seeking a **Continuing Care Specialist** to join our team in **Quebec/Ontario East**.

Reporting to the Continuing Care Manager, the Continuing Care Specialist owns the patient relationship once they have graduated from the ongoing care of the Clinician and are accountable to ensure the patient has everything, they need to ensure ideal patient outcomes through follow up and education on our products and services.

Key Accountabilities
- Contacting patients to check in on how they are progressing with their treatment and discuss ongoing equipment needs to ensure highest level of care and optimal therapy.
- Processing orders of new equipment for the patients.
- Booking appointments with Clinicians where patients express concern with their treatment program.

Specific Accountabilities
- Maintaining patient database to ensure timely follow up.
- Call patients to assess their needs, ensuring their equipment is working properly and replaced within the approved manufacturer’s guidelines.
- Discuss cost of service, insurance coverage, and payment options with the patient.
- Liaise with insurance companies on behalf of and/or in conjunction with patients in order to determine insurance entitlements.
- Reviewing clinical data with patients and discussing specific therapeutic issues as required.
- Confidentially manage patient accounts.
- Work as a team player to ensure each patient receives the best service possible.
- Keep current on various products and services from our vendors.
- Have the ability to perform basic clinical data interpretation.
- Perform other related duties as assigned.

**Nous sommes présentement à la recherche d’un(e) spécialiste en soins continus pour joindre notre équipe couvrant le Québec et l’est Ontarien.**

Relevant du responsable des soins continus, le spécialiste sera responsable du maintien ainsi que du suivi tout au long de la progression du traitement de chaque patient afin d’assurer le renouvellement des accessoires nécessaires grâce à l’éducation continu sur nos produits et services.

**Principales responsabilités**
- Contacter les patients afin de vérifier la progression de leur traitement ainsi que discuter le renouvellement des accessoires dans le but d’assurer le plus haut niveau de soins qui assurera une thérapie optimale.
- Sera responsable de générer les commandes de nouveaux équipements pour les patients lors de chaque suivi.
- Prise de rendez-vous avec les cliniciens lorsque les patients expriment des préoccupations concernant leur traitement.

**Responsabilités du poste**
- Tenir à jour la base de données des patients afin d’assurer le suivi en temps opportun.
- Appeler les patients afin d’évaluer leurs besoins, s’assurer que les accessoires soient renouvelés ainsi que vérifier le bon fonctionnement des appareils conformément aux directives du fabricant.
- Répondre aux appels, courriels et questions des patients, ainsi qu’acheminer et faire le suivi des soumissions auprès des diverses compagnies d'assurances.
- Discuter auprès du patient, du coût lié à chacun des produits, de la couverture d’assurance et des options de paiement offertes.
